On your appointment date, it is very important that you bring with you a picture ID, verification of insurance, and complete the following forms:  Patient information form, authorization consent, medical history form, medication list, and practice financial policy .  We have requested from your referring physician medical records pertaining to this consultation.  Please contact your referring physician's office to assure the records have been sent prior to your appointment.

Upon scheduling your appointment, we require you to verify your appointment at least 72 hours (3 days) prior to your visit.  Please contact our appointment verification line at 587-6010 ext 239 (24hrs a day) to confirm your appointment.  This allows us an opportunity to offer this time to someone else should you need to cancel this appointment.

Our office will file your primary insurance.  Please be prepared to pay your portion of the fee not covered by your insurance at the time of you office visit.  Please feel free to request information about our fee schedule before or at the time of the visit.  If there is a question regarding our financial policy or your account, our business office is prepared to assist you.  This include copays, deductibles and co-insurance.  Insurance and eligibilty is verified prior to the visit.
